Dear District 146 Families:
Illinois State Law requires all students entering Kindergarten and Sixth grade to submit a complete physical examination and all required immunizations. The Illinois Department of Public Health Certificate of Child Health Examination form is located on
our district website. Medical forms are also available at any provider’s office where immunizations are administered.
Reflecting State of Illinois requirements, District 146 school board policy reads: A student’s parent/guardian shall present proof that a student received a health examination and the immunizations against, and screenings for, preventable communicable diseases, as required by Illinois Department of Public Health, prior to entry to:
- Kindergarten or First grade (if child did NOT attend Kindergarten)
- Entering Sixth grade; and
- Enrolling in an Illinois School, regardless of the student’s grade (including nursery school, special education or secondary schools, and the students transferring into Illinois from out of state or country.) This includes pre-school students receiving speech itinerant services.
Physical Exam must be dated within one year of entering the grade. For a full list of health forms required please refer to

Please understand failure to comply with the above requirements by October 15 of the school year will result in your students’ exclusion from school until the required physical exam and immunization records are provided to the school nurse.
